Managed Care is a valuable service offered to members by Transmed. Please click on the Managed Care submenu above to access the Chronic Medicine Management section, which includes the Chronic Medicine Programme reference guide, application form and formularies. The Disease Management section provides members with the opportunity to view and print the Disease Management Programme enrolment form and various health leaflets.
